Registering your details before your appointment

To prepare for your first vaccination appointment, you can enter your personal details on the following webpages and provide information on your personal medical history by completing a brief questionnaire. The data you enter in this process will only be stored on your own computer; it will not be sent to an external server or forwarded to a third-party.

After filling in the questionnaire, please print it on a sheet of A4 paper or save it as a PDF file. You should then bring the print-out of the completed questionnaire or the PDF version on a smartphone or tablet to your vaccination appointment. The team there will be able to read the data automatically using the QR code generated when you created the document.

This is a voluntary process which will help speed up your vaccination appointment. If you are unable to register your details in this way prior to your appointment, this process will simply be carried out on-site before you get your vaccination.

• To access the pre-registration process (by clicking “Fill out registration form before appointment”)you will need to ensure your browser software is up-to-date.
• Clicking on Fill out registration form before appointment or Barrier-free access to registration form will take you to the site where you can fill out your details prior to your appointment (esQlab website for vaccinations created by the company gradient.Systemintegration).
• When you close your browser, the cache and your data will be deleted, assuming the default settings haven’t been changed. You can also manually delete your data in the Patient List tab at any time.
